* E-mail address. We shall use this address to inform you of action on your request.

* Course number, NOT A CRN. ONE COURSE PER FORM. For the courses numbered 19000, 29000, 39000, 49000, 59000, 59100, and 690900, append to the course number one space and then the three-character section identifier. What you enter should look like this "18000" or like this "49000 001" (without the quote marks). Any other format or additional characters may cause your request to be ignored.
